Terms and conditions outlined - Diamond Post Care is a recovery service that assists in facilitating and monitoring my recovery, as per my doctor's discharge instructions. Transportation Policy Check-In: 3:00 P.m. **If surgery is the same day that is subject to change Check-Out: 11:30 a.m. Diamond Post Care offers Transportation, this service starts the day of your Check-in from 7am - 5pm ***subject to change for SURGERY, AND 1ST POST-OP unless authorized by an associate. Transportations Starts on the day of CHECK- IN. Surgery Check in Day: if you are to check in on surgery day, you are required to check in with us an hour before your time of surgery. If you have additional appointments and would like us to transport you, please contact once you have received notification for the next day. Transportation requests must be completed prior to 9:00pm the day before in order to be guaranteed transportation. there will be a $25 dollar roundtrip fee. subject to approval. Transportation is subject to a max 10 miles, one-way. ***subject to change for SURGERY, AND 1ST POST-OP unless authorized by an associate. there will be an extra charge of $25 per 10-mile radius. The prescriptions are taken to the pharmacy closest to the recuperation home. Monday through Saturday, between 7am and 5pm, pharmacy pick-up and drop-off will be available. You are responsible for providing identification and a method of payment to cover the cost of your prescription. **** If you want to utilize your own pharmacy, you are responsible for submitting prescriptions and retrieving medications. Check-in time is 3:00 pm and Check-out time is 11:30 am, Monday through Sunday. If you choose to check-in or check-out on Sunday, you are responsible for your own transportation. A late check-out is only allowed if the house is not entirely occupied and, if approved, you will have to pay $60.00 for three-hour and cover the cost of your own transportation. If it is something we are unable to do, we will let you know. You are subject to receiving transportation from a Lyft / Uber. At any time of your visit, please contact customer service/transportation to be picked up. Meals During your stay at Diamond Post Care, we include three meals per day at no additional cost. *** Breakfast, Lunch, and Dinner, snacks and drinks . In addition, we facilitate dietary restrictions and allergies for an extra fee. No eating is permitted in the rooms. ❖ Diamond Post Care does not cater to any preferences and is unable to make changes to their menu upon your check-in _ __________ I agree to disclose all my dietary restrictions/allergies prior to my arrival. If the information is not given prior to my arrival, I will need to provide my own meals. Meal times Breakfast 8:30 am Lunch: 12:30pm Dinner: 5:30pm Code of Conduct ❖ Diamond Post Care promotes an environment that tailors to their clients’ recovery. We require that you speak with a low voice inside our facility and be considerate of our clients and staff. We do not allow any profane or vulgar behavior inside or outside of the premises. We require that you are respectful towards all our guests and staff at all times. You must use headphones during any call. We do not allow the use of a FaceTime / speaker phone since we strive to maintain a peaceful environment in our facility. Failure to follow these regulations will result in removal from our facility and your money will not be refunded. Canceling & Rescheduling ❖ Diamond Post Care strives to accommodate the needs of our clients, especially when it comes to cancellations and rescheduling. If you need to change the date of your surgery, please let us know prior to your check-in. Doing so allows us to rearrange your reservation without the risk of losing your deposit. You will need to speak to one of our associates with the NEW DATES a month prior to ensure availability. Before choosing a date with your clinic you would have to contact us to give you the dates available. ❖ We do not guarantee any dates to RESCHEDULES . In the event that you can’t stay due to a surgery cancellation or a last minute change, we do not refund deposits or any other payments (this includes massage deposits, services, and other merchandise). Refunds and Payment Policy A $300 NON-REFUNDABLE DEPOSIT (— $300 / nightly Rate ) locks in the dates you have requested from us. This deposit is NON-REFUNDABLE and will be deducted from your total balance. Your money is secure with us for two years, in case you need to reschedule. If in the event you decide to not go through with your surgery you can transfer your deposit to a friend/family or use your deposit to make purchases for in house items. *Transfers require your written permission via email. ***The difference will not be refunded if you do not stay the complete number of days after making and confirming your reservation. If you choose not to stay with us but continue to have massages at our facilities, this will be considered full usage of the services. *****IV DRIPS, Massages, and Promotional Packages are non-transferable services or products, as well as any exchanges with members of the house. Failing to confirm your reservation one week in advance will result in automatic cancellation. Deposits and payments will be forfeited if you cancel a reservation that has been reserved specifically for you. Purchased massages and items cannot be returned for a refund. Items are Final sale. ❖ Deposits are to be submitted using QuickBooks Intuit Online processing system. Remaining balance is due upon arrival using a U.S. postal service Money Order Cashier's Check, or cash made PAYABLE To Diamond Post Care Inc.
